PHARMACY VOUCHERS AVAILABLE FOR PATIENTS IN NEED
The Community Foundation of Dunn County is offering pharmacy vouchers for underserved and uninsured individuals in Dunn County and surrounding areas related to diabetes and cancer care.
The Cammie B. Johnson Diabetic Care Fund was established by the family of Cammie B. Johnson, a young woman who passed away just before her 21st birthday from complications of diabetes. Each year, the Diabetic Care Fund issues pharmacy vouchers to residents in and around Dunn County living with Type 1 or Type II Diabetes who are experiencing difficulties accessing supplies because they are low income, uninsured, between insurances, or have insurance but cannot currently cover their out-of-pocket expenses. Vouchers of up to $500 each are available. Limit (2) vouchers per applicant per year.
The Garding Against Cancer Fund was established by UW-Madison Men’s Basketball Head Coach, Greg Gard, and his wife Michelle. Coach Gard lost his father to brain cancer in 2015 and they have since created the Garding Against Cancer initiative, part of the Wisconsin Foundation and Alumni Association. All money raised stays in the state of Wisconsin for cancer research and patient care. In September 2017, a Garding Against Cancer event was held in Menomonie, along with UW-Stout, in advance of their exhibition basketball game at the Kohl Center, and proceeds from the events are being used to provide cancer patients who are experiencing difficulties accessing medication with pharmacy vouchers, up to $500.
Eligible patients in need simply complete the voucher form upon request at participating pharmacies to receive a voucher to purchase the medication and/or supplies they need related to diabetes or cancer, up to $500 per voucher. Limit (2) vouchers per applicant per year. There is no wait period or approval process. Patients in need can have their prescriptions filled same-day and any out of pocket amount they owe but are unable to afford, up to $500, will be invoiced to the respective fund of the Community Foundation of Dunn County.
